Memory Care Director

Job Summary

Responsible to plan, organize, and implement the dementia care program for the facility according to the federal, state and local guidelines, regulations, and facility policies.

Requirements

*Have an associate or bachelor’s degree in social work or be a registered nurse

*Must have a least 2 years’ experience in a supervisory capacity in a hospital, nursing care facility or other related medical facility

*Have at least 1 year experience working with persons with Alzheimer’s disease and other dementia

*The unit director shall obtain at least 12 hours of continuing education every year, especially related to serving residents with Alzheimer’s disease and other dementia

Responsibilities

*Assesses patient characteristics, including, cognitive and functional abilities, Alzheimer’s dementia stage and prior lifestyle

*Participates in the initial pre-admission evaluation of potential patients

*Evaluate the effectiveness of individual behavior management interventions utilizing nursing documentation, incident reports, flow sheets, checklists, etc.

*Collaborate with the Interdisciplinary team to develop individualized resident-centered plans of care

*Monitor the resident’s environment for changes

*Coordinate and facilitate programs for the memory care unit that are engaging and appropriate for residents at varying stages of cognitive decline

*Works with staff members to provide Alzheimer’s and Dementia in-service trainings

*Conducts appropriate reviews of the memory unit to ensure compliance with program standards

*Ensures appropriate discharge plans are available upon admission and revised if necessary
